In an emergency if Civic Park Consulting Suites is closed, please call 000 or present to the nearest hospital to you, with an emergency department. A home visit MAY be available after clinic hours (where Doctor deems appropriate). Please speak to the Receptionist who will discuss further with the Doctor.
For non medicare card holders, private charges do apply. Please speak to Reception if you would like to know more.
Our Receptionists are not authorised to discuss your results with you. Patients are required to make an appointment to discuss results with a Doctor.
Civic Park Consulting Suites complies with the Health Records Act (Vic. 2001) and the Privacy Act (Commonwealth 1988). Your medical record is a confidential document. It is the policy of this Practice to maintain security of personal health information at all times and to ensure that this information is only available to authorized persons involved in patient care. If you wish to transfer your records, you will be asked to complete an information release form, and to provide us with photo ID that we can copy. This is to maintain privacy and confidentiality of your records.
Communication with patients via electronic means is conducted with appropriate regard to privacy. Patient consent is obtained and documented. Whilst not encouraged, Civic Park Consulting Suites allows patients an opportunity to obtain advice or information related to their care by electronic means. Civic Park Consulting Suites will only provide information that is of a general, non-urgent nature and will not initiate electronic communication (other than SMS appointment reminders) with patients. Any electronic communication received from patients is also used as a method to verify the contact details we have recorded on file are correct and up-to-date. Civic Park Consulting Suites uses an email disclaimer notice on outgoing emails that are affiliated with Civic Park Consulting Suites. Please also note that our receptionists are not medically trained to give advice over the phone. You may ask to speak with a doctor in case of an emergency. Where possible, your call will be put through to the Doctor or, if unavailable, to the Practice Nurse.
